Order Form

Our order form is available for you to download (this file is in PDF format, 314Kb). Page 1 contains sections for you to complete; page 2 contains instructions for using the form. If you are printing the order form to mail to us, you need only print the first page.

You may either print a copy of the form for mailing to us, or fill out the form using your computer and save a copy that can be uploaded with your order. Note that if you fill it out on a computer, you will need the full version of Adobe Acrobat, or the very latest version of Adobe Acrobat Reader. If you want to send your order online, we recommend that your just put your contact information and requirements in a text file, and ignore the order form.

However, we need the following information provided in the order form for every order:

• Your full name, address and email contact details.
• The type of work you require.
• Clearly defined details of any special requirements or preferences you have for your project (such as level of detail for sequencing or transcription).

But don't worry too much about these items. As long as we have your email address, we will contact you to obtain any missing details.
